It always has to do with bad team selection, wrong manager, or by premature retirement of players who can still make a good contribution to the National team but refused to play for England.
In the last WC, the Manager made major blunders by selecting the wrong players and leaving good players on the bench. A good example was, selecting Raheem Sterling over Jamie Vardy who is a better finisher and has the same speed as Raheem himself in the last WC in Russia. After the WC Jammie Vardy retired from England.

Ward prowse is not disrespected he was just worse than other central midfield options. You can take set pieces from the bench, there is enough options for set piece takers that being good at them isn’t enough to justify his selection
I know you mentioned him for his lacklustre coaching, but Glen Hoddle deserves a mention because he got only 50-odd caps whereas all the players in mainland Europe and in South America considered him the best English midfielder of his time.

disagree with Trent Alexander-Arnold being included. He’s in an era where England have 4 or 5 top level right back options. It makes sense that he’s rotated. He was also bad the season before Euro 2021
